In 2026, Nepal is projected to have a strongly female-skewed sex ratio of 90.68 males per 100 females, indicating that women outnumber men by a clear margin. Based on the overall female-to-male ratio, Nepal ranks 24th out of 237 countries and territories worldwide and fifth out of 51 in Asia, placing first in the South Asia region. Its gender ratio is comparable to that of Antigua and Barbuda, Portugal, and Estonia.

There are 14,090,565 (14.09 million) males and 15,538,845 (15.54 million) females in Nepal, resulting in a female surplus of about 1.45 million people. In percentage terms, females make up 52.44% of the population, compared to 47.56% males. Nepal ranks 20th globally in terms of absolute female surplus and fifth within Asia.

In Nepal, the male-to-female ratio rose from 99.01 in 1950 to a peak of 103.62 in 1985, with men outnumbering women from 1958 to 2004, before declining to a projected 90.27 males per 100 females by 2028.

The sex ratio at birth in Nepal is approximately 104.9 male births per 100 female births. Historically, it has remained relatively stable, fluctuating within a narrow range from about 104.3 in 1985 to 105.5 in 2015, indicating minimal variation over time.

As of 2026, Nepal has more males than females in age groups under 20. The highest male-to-female ratio is found in the 10–14 age group, with 106.62 males for every 100 females. For children under 15, the sex ratio is 104.93, while in the working-age population (15–64), the ratio is slightly male-skewed at 85.94 males per 100 females. However, the ratio drops significantly in those over 65, with only 83.79 males for every 100 females. Among the elderly, women greatly outnumber men, with nearly 60% of those in the 85–89 age group being female, and a 3-to-1 ratio of women to men among centenarians.
